  • How will my eligibility for services be evaluated?

    • Meet the functional criteria for a developmental disability.

    • Intake process with DDD.

    • U.S. citizen and resident of New Jersey

    • Are eligible for or enrolled in Medicaid.

  • Eligible now for DDD Services- Next steps:

    Nj state will provide:

    • List of the service providers/ Support coordination agencies. (You can choose 1)

    • Received Letter of eligibility.

    • Choose your support coordination agency to start services.

  • Interviewing Support Coordination Agencies.

    What is a Support Coordination Agency?

    An organization that will help you get the services you need.

    • Select a support coordination agency of your choice.

    • (Required to choose at least 1)

    • Fill out the Support Coordination Agency Selection Form.

  • Support Coordination Agency

    • Contact us to begin the process of the ISP plan. (Individual Specialized plan)

    • Choose Direct Support Service Agency For your plan.

    • Contact us to help you step by step to become a live-in caregiver for your family. Other services are available.
